CONSENT TO THE PROCESSING OF PERSONAL DATA

 Adapted to the provisions of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, (GDPR)

 Affected person: Customer / consumer Operator: Centrum Jonas s.r.o., Tolstého 5, 811 06 Bratislava, ID 47 555 700 

List of personal data: Name, e-mail address, date, place of birth, place of use of the calculation, data about pregnancy. 

Purposes of personal data processing: 

 Inclusion of personal data to relevant database of provider to create a login account, made of and management of delivered documents. 

 Personal data must not be further processed in a manner that is incompatible with those purposes. 

Recipients of personal data: Centrum Jonas s.r.o. 

Legal basis for the processing of personal data - Article 6, para. 1. letter a) GDPR - the data subject has consented to the processing of his / her personal data for at least one specific purpose. 

Retention period of personal data - 10 years. 

 The data subject hereby declares on her honor that she has given the controller her express and unconditional consent to process his personal data to the extent specified above, for the purpose and for the period of time specified above. 

 The data subject is obliged to provide true and actual personal data. In the event of changes of personal data, the data subject is obliged to notify the controller of the change without delay. 

 The concerned person has the right to withdraw her consent at any time. 

Withdrawal of consent shall not affect the lawfulness of processing based on consent before its withdrawal. 

 The rights of the data subject are defined in Chapter 3 of the GDPR. It goes e.g. o the right to request from the controller access to personal data concerning the data subject, the right to rectify or delete them or to restrict processing or the right to object to the processing, as well as the right to data portability and the right to lodge a complaint with the supervisory authority. 

 By ticking the checkbox, the concerned person confirms that she has been informed of the information provided and agrees with it.
